Your out-of-pocket cost depends on your plan and how much you’ve already spent this year. For most [insurance name] policies your responsibility is the sum of your remaining deductible and your co-insurance up to your out-of-pocket maximum. Policy deductible – the amount you must pay out of your pocket each year before your insurance benefits kick in. Until you reach your deductible your insurance pays zero. Deductibles can be individual or by family.
Deductible already met – the amount you have paid this year toward your deductible.
Out of pocket maximum – the most you can be expected to pay each year. After you reach your out-of-pocket maximum your insurance picks up the remainder of the tab.
Out of pocket already met – the amount you have paid toward your out-of-pocket maximum. This includes co-insurance over your deductible.
Co-insurance – the percentage of charges you are responsible for above your deductible but below your out-of-pocket maximum.
