Aetna was founded in 1873 and is headquartered in Hartford, Connecticut. Aetna is now a member of the CVS Health group. It is the nation’s third-largest provider of health insurance and related services, and provides health, dental, vision, and Medicare plans to individuals and employer groups. It has received high ratings from AM Best and an A+ rating from the BBB.

Aetna’s Health, dental, and vision claims are typically filed by the provider. Claims are paid according to the benefits spelled out in your specific plan. After the provider submits the claim, Aetna will pay according to your plan. You’ll receive an explanation of benefits (EOB) describing what has been paid by Aetna, and what, if anything, you owe. You’ll receive a bill from your provider for any balance to be paid by you.

How much will the surgery center cost under my plan?

Your out-of-pocket cost for the Surgery Center depends on your plan and how much you’ve already spent this year. For most Aetna Health Insurance policies your responsibility is the sum of your remaining deductible and your co-insurance up to your out-of-pocket maximum. To estimate your Surgery Center related out-of-pocket cost (expense) for a procedure under your Aetna Health Insurance please complete the following:

Policy Deductible ($) i the amount you must pay out of your pocket each year before your insurance benefits kick in. Until you reach your deductible your insurance pays zero. Deductibles can be individual or by family.
Deductible already met ($) i the amount you have paid this year toward your deductible.
Out of pocket (OOP) maximum ($) i the most you can be expected to pay each year. After you reach your out-of-pocket maximum your insurance picks up the remainder of the tab.
OOP already met ($) i the amount you have paid toward your out-of-pocket maximum. This includes co-insurance over your deductible.
Co-insurance (%) i the percentage of charges you are responsible for above your deductible but below your out-of-pocket maximum.
